Before your first session, please download, print and complete the Child/Adult intake form and have it available for the session. (Forms can also be emailed upon request). This will save us time and allow us to immediately focus on you and/or your child!
Please bring in a copy of any previous evaluation reports. They can help us better diagnose your child. These reports can be e-mailed to us at [email protected]
